Most extreme, maybe. Not the most exciting to watch tho.
Isle of Man TT is insanity disguised as a race and it's extreme in a sense that all it takes is a mistake for you to crash into a tree at 200 mph or 320 kph, which is instant death for bikers; any WEC race with the LMP1 cars are insane as well just for the acceleration and speed cornering (which is a lot nicer to watch than F1 imo) and you also have the Formula E which is hilarious because the drivers are the definition of scumbag at times: D
Anyhow, I can see why some people watch and enjoy this.

the US has its own version of this, in the southern states they have rock bouncer competitions which is essentially this but instead of dirt they climb up rock slopes

It is not twice as steep as the worlds steepest public road. Kirkstone pass in England is 33% and there's way steeper ones than that.
