Only reason why some French do not speak English is just that they simply refuse to. They are so nationalistic that it is annoying. The thing you should do is just leave the place where they refuse to speak language you can speak. It means they do not respect you as a customer and think their culture is so special that they force it on others. Besides, they refuse to speak other languages in other countries too so I see no harm giving it back to them.

#1 is completely wrong. I've found that most French business owners, servers, etc. have very little patience for Americans attempting to very slowly butcher their language. Most French people would much rather just bite the bullet and speak English in order to move the conversation/interaction along (especially if there's a line.
