I made a form and added a quested where a supervisor approve a task, is there any option that someone request IT Assistance and then the supervisor (only one person) can approve if the requester can get assistance or not how can I restrict other staff from click approved check box while it should be clicked by supervisor only ------

Is there a way to create the form with a button that says something like -Add Item?- I currently have a spreadsheet and need clients to input the names and information for their employees. The issue is I need to be able to Add Item or Add Employee since many have different amounts of employees.

This is a great tutorial.
I have a question about functionality: is there a way to record the time the survey was opened and how long it took to submit it IN ADDITION to the time stamp of when it was sent?

Don't you have to create branching to keep those on page 2 from going to page 3? If they branch to page 2 or 3, won't they miss your scale question?

Can Google form be used as a back-end and it have a back-up to a database. I mean if the Google form data can be link and joined to the database
