If you can find a first print of The Beastmaster-a moat filled with oil was covered with something brown (soil. Anyway, when a torch was tossed in the moat was supposed to catch fire. Instead the moat EXPLODED. In the first print you see the camera shake from the shockwave. Also, the actors were next to the moat which not only blew up but fireballs were raining down and the actors had to avoid those while still remaining in character-fighting next to the moat.

Oliver Robins from the first Poltergeist: the clown doll that strangles his character from under the bed malfunctioned!
Everyone else thought he was just acting REALLY, REALLY well and selling it, but it wasn't until he started going blue in the face from hypoxia that the realisation set in that he wasn't acting: he REALLY was being strangled!

Actors performing stunts should have some sort of signal they can give if things go wrong. If one isn't pre-arranged, flipping off the camera will, in most cases and movies, ruin the take, causing a cut, and they will let you get some air before the next take

This story about Ed Harris is new to me but there is another from the same film where his character is descending into the abyss and crashes into a rock face. Evidently that scene was real and Harris was injured and in real danger.

You simply can't make a list like that, without mentioning the true legend Jackie Chan. The guy broke every single bone in his body(including his skull) and was also electrocuted.
