I think the 'fatal disease' Jacob is referencing in the title isn't the disease his friend believed he had, it's actually a 'disease' everyone has of not living life to the fullest and taking opportunities. People only really begin to live life if something heartbreaking happens to them, because they realise what they have/could have lost. For example in this case, Jacob's friend believed there was a huge possibility he could die, which probably allowed his friend to see life for what it truly is.

Some of the reality is real, you have to balance the truth. We don't have all that power only some otherwise we would prevent death, violence and so on. Some of this is true with self esteem, got to eat drink pay bills, that is reality, you can, t think that away
