Well with Rick being in the hospital, I have two theories and for some this may be a spoiler.
He is either still in a coma and dreaming all of this, because later on in the episodes he actually dreams that he is still in the hospital.
My other theory could be that even after only 2 months, the power could still be on and the generators for the hospital is on. When he walks through the hospital, there is a light flashing on and off. So even though we can't hear the heart beat monitor, and or the IV is being gravity fed into his veins which is giving him the fluid to survive. Now, it could be with that lady you talked about changing out his IV bags too.

OK. Approx 2 months since the outbreak BEGAN, not when it had progressed to the total collapse of the world as we know it. There are many factors that could stretch out that span of time between the start and the collapse. (Or, I guess, when it went down at that hospital)

Ugh, The first two episodes, one of which, the guy with a kid (black guy who has badass ninja skills) says that the infection has been going on for a specified period i dont remeber exactly, but its in the first two.
