i tend to think this movie is pretty straight forward and honest in how it was shown, he really was a crazy guy who was given free reign for a few days to shake him out of his fantasy world and i think it worked but he couldn't handle knowing the truth anymore so when his psychiatrist came to him on the stairs he called him chuck and pretended to still be crazy so they would give him a lobotomy. this is evidenced by the fact that he asked that final question which he never would have as teddy, and then when chuck called him teddy he didn't respond, if he was still crazy he would have played along with the fantasy of being called teddy.
also when chuck says they're too smart for them you can see him pause and think about it, and he agrees because he knows that he's going to get what he wants, which is a lobotomy because he's too smart for them and he's tricking them into thinking he's still crazy. i might be reading too much into that, but i don't think so, i think the movie was brilliant the entire way through but that final scenes steals the show for me, it's shot so well and performed amazingly well to top it off. insanely underrated movie.

I'm a Psych major although I didn't practice clinical Psychology but I love watching this kind of movies. Just my 2 cents.
Remember when he was in the cave with Rachel? And Rachel told him about these test and ghosts that NK have done to its people and its soldiers before spreading them around the globe? I believe that the US are doing an experiment in that Shutter Island to test if they can truly alter or totally make a new memory on one's mind so they can control it like what NK did. Leo I believe was truly TEDDY (US Marshall) that was too close to know the inhumane experiments happening in that island that's why they made him believe that he is crazy (w/c in fact somehow they succeeded) by making him to hallucinate having false images/narratives of his wife and kids (which they pushed more during the confrontation in the lighthouse)
The last scene I believe indicated that he were tuly Teddy and called Dr. Sheen Chuck because he accepted his fate that he will never ever be able to leave the island so he pretended and play along for them to think that they failed. Thus he asked Dr. Sheen which is better, to live as a monster (like Dr. Sheen making someone crazy) or to die as a good man (Teddy as he knows those accusations of murder are false just for him to feel guilty and altering and tampering his memories to make him crazy. He chose the latter bcoz he doesn't want to live like the monster (Andrew Daniels as murderer) and shoulder a false guilt of an event that he didn't commit so just they could control him.

I am convinced that this is what happened: Teddy was completely sane, and he really was sent to the island to investigate. after finding out that there is terrible things happening on the island the boss couldnt let him go and he had to find a way to stop teddy. When Chuck later in the movie disapears I think that he was taken by the docs on the Island and tortured and manipulated to think and say that he also is one of them. the scene in the film where teddy is talking to the boss. and the boss says that he came alone and had no partner is the boss trying to make it look like teddy is insane. but teddy is smart and says himself that he has no partner.
Teddy and chuck was completely sane, they came to the island to investigate. but after finding out that there were terrible things happening on the island the boss had to stop them. He managed to torture and manipulate chuck, and tried to make teddy think that he also is insane so that he can make him a pasient of the island. although teddy never becomes insane he is still trapped on the island because of the bosses methods. in the end scene he has the conversation to chuck and says that it is better to die a hero than live like an insane. he walks off, and in the next scene you see the ocean crushing inte the rocks. i believe that afte the conversation with chuck he walks off and jumps down the rocks killing himself. realising that there is no way back.

Teddy actually knows some secret about shutter Island and something fishy is going on. And he also knows that andrew laeddis is there. So he volunteer to go there. Since this whole Island is run by government they try to eliminate any kind of distraction that would cause trouble to their program which is the island. So they place chuck as teddy's partner who is an actual doctor or government ajent or ajent of shutter Island because he offers teddy cigrettes on the ferry and says something like government people easily steals your stuff or something! And offers his own cigrettes which are drugged. According to me story of rachel is actually true. That she was a nurse and the whole thing of her being lost was setup by the island because she too knew something and was potential gonna expose the whole experiment. Teddy was being drugged and wasn't mad can be proved when the lady writes run only when chuck leaves the table because she knew he's a doctor. And the 67th inmate isn't actually Andrew leaddis but the person teddy talks with whos locked in the cell. And teddy decides to get killed because in the caves Rachel tells him no matter what you do once you have been declared mad no one's gonna believe you.

Hes so traumatised by his this (who wouldnt be) that he goes nuts himself.
He creates a story where his wife died his children dont exist and hes just getting on with his life doing his job.
But shit dont makes sense in his story and hes trained, highly intelligent and dangerous and unintentionally he is causing harm.
Everyone else is part of the mental hospital and is trying to help him realise the truth that hes buried deep inside his mind so they they all act like theyre just part of his story.
It works and they bring him back.
But he realises at that point that the gravity and trauma of what hes done and what his wife did with his kids. Hes got nothing to live for. How could you live your life having gone through that. He would be a prisoner for murdering his wife and every day would think about his kids and his and wife and what happened. Hes better of dying and his words at the end he kind of explains hed rather die/commit suicide and being lobotomised is the equivalent of that.
Amazing psychological film I rate it 10/10

I reckon that he is Andrew Latis as their is loads of things suggesting that he started the fire in the apartment:
Lighting matches in ward C
Lighting Drs car on fire
His wifes ashes being in his hands when he has a flash back to her (I think its the first flash-back)
However, I think the Drs have been making him worse as he seems normal at the start of the film (when arriving at Shutter Island) but then gradually gets worse throughout the film:
Shouting/screaming
Strangling people
Always hunting round the island which is suggesting he has escaped
Also, can u remember he REFUSES to take the pills when offered them when he gets sensitive to the light from the thunder all of a sudden? - I think he knows hes getting worse.
The Lighthouse at the end is a big giveaway (I think youll have noticed) that hes going for a labotamy. Subsequently, everyone has been playing along with his fantasies to try and get info out of him as, at the end of the film, Leicester Sheen references him as boss and what are we gonna do next
- In other words he has committed crimes but they have only worsened since he arrived on the island

Theres only one explanation to this movie and its the computer god aka the all seeing eye. (why he wants ppl to speak out openly about him but ppl refuse to IDK but) the cops started off good wanting to investigate a crime, but while investigating it they uncovered more things that they would have told the world about so they had to be stopped by these bad ppl. If the cops would have never gave in like they did they wouldve been set free because how could the shutter island explain a fake made up story to the rest of the world, therefore the truth would have came out! The cops where forced to live a lie and willingly accepted it even tho it would mean that they spend the rest of there life in a mental hospital. the cops should have never gave up they where going to get freed that next day and could have told everybody what they where doing on that island! And at the beginning he took medication getting off the boat then the old lady told him to be quiet because if you gone tell in them they gone try and get you to look crazy

Somethings should be taken at face value. That being said, hyper-analyzing the burning woman was done poorly. You can see clearly that once she dissolves into ash; he is left with nothing but water in his hands. This is reality setting in under the illusion. The only reason the water was leaking onto him when he awoke, was to throw off the audience. Furthermore, the warden was talking about his hyper violence because of the war crimes and outbursts against patients. The flame in the cell scene was because, up until that point, we were led to believe that the culprit that burned his house down was there. There was a bit of trickery in the metaphor. Lastly, subtle nuances between how Andrew and the staff/residents interacted were enough to correlate that they were indeed, previously familiar with who he was and what he was doing.

Instead, he uses that to eliminate only marshal investigating the crime inside. He baits him, he places that 67. And code of 4 paper there just to convice him that his story is real. He was marshal before Shutter, and as its said, it's his first time working with Chuck. He may read somewhere he is from Portland, but Chuck claims he is from Seattle. Doctor Seehan dissapears right before Chuck begins to exist. Long story short, I think Leo got brainwashed after watching this movie fof 5th time.

And yupp who knows I might be wrong here. interpreting everything. but I love the way how the film got us thinking about what was the actual plot. what was real. SHOULD MAKE A 2ND PART. I presume. it would be very convincing for the audience that have watched the part 1. to get their own plot, of which is real and which isn't, to be cleared!
