Bran getting pushed by the tower was shocking but i feel like the best plot twist for the first episode is the fact that incest is considered a normal thing.
When i saw Cercei and Jamie i was in so much shock, it was like the first hint to show us that its not a normal tv show.
If brutal deaths and betrayel was not enough, we also got incest, i dont want to be an asshole or anything but i gor to say, we live in a really weird world if a tv show where incest is considered a normal is called the best tv show of all time, i mean it is good and all but Breaking Bad has more realistic and better storytelling and of course a satisfying conslusion.

The twist of Friday Night Lights is still one of my favorites to this day. You watch the first half of the episode thinking it is going to be all about this star player and then suddenly, his entire life changes in a blink. It's absolutely brilliant and sets up the course of the rest of the show.
