Video game NPCs don't always have it easy. It's a pretty lonely digital life. Here are the worst examples Dakuu75: I remember in the FFXI MMO back in San'dOria there was a kid NPC running around all the time doing mail deliveries I guess for pay in order to help support his care-giver a ricketty old grandma dying of of some wierd disease (which you later get the quest to cure her of). That kid... Like I hanged out in San'dOria A-LOT that was my home-town in FFXI and in -south sandy- (That's souther San'dOria for you normal folk) he would pass by all the time doing the same routs never taking a break... I mean I get it, -NPC- and all but, I still felt bad for the kid.

Not exactly a gaming NPC, but I know a character that was stuck in a box for 19 years, with two weeks worth of supplies (barely managed due to certain reasons), entirely isolated in said box which had One way audio and impossible to make noise out to be heard outside. So naturally several times she had the potential to be rescued, but said box was too tough to break and thus 19 year long solitude until she got 'rescued' by her aunt.
It's a long story, and trust me when I have 'rescued' like that.

Brandis in Fallout 4 really shows how much the player is so vastly on a different level with everyone else in the game. ---Spoilers--- They had a whole group of armed, power armored Brotherhood members and they got completely wiped out of existence by a few dozen Super Mutants. The Player shows up and just puts them all to shame. Still a sad lonely story for Brandis, but they couldn't just.. you know... win? Life is hard for an NPC.

Lucia from whiterun. She's a beggar in whiterun and just a child. She's an orphan who only has one friend, another beggar with a drinking problem, and everyone thinks that the reason she follows everyone around is to steal from them, when she just wants to be adopted. So even though she lives in the middle of a city, she is very lonely.

Morkvarg in witcher 3 he used to be a bad guy, and then he had been cursed to werewolf and had suffered loneliness and hunger, can't even eat ... when talk to him u will feel that (good voice acting) although he was a bad guy as i said but i decided to lift his curse and let him leave and live out

Lucatiel of Mirrah.... Maybe a bit too late or someone already mentioned her. I just met her in Aldia's Keep right now in my second playthrough, I played through her quest, because it pretty much gives the vibe of loneliness.... And confusion..... And desperation....

A homeless guy in a town just wanting a conversation. You return years later to the same town; he's there, but hidden. He explains the city told him to hide because no one wants to see him, he's too ugly a sight when you see the town

How about dodos from ark survival evolved , everyone loves you but they constantly abuse your slowness and small size, oh and your second to the bottom of the food chain - usually your on your own and players keep killing all your friends.

I'm wondering what about the npc arena fan who basically just waits for the player character to show up and if he dies he respawns in another town but seems to have no home and does not interact with anyone else he is from oblivion

Kinda disappointed not to see the Hollow Knight here. Guy was alone for who knows how long in that prison of his. Kinda sucks that he is self-aware--ahem, impure--too.
Also Veronica Santangelo's endings are all a bit sad.
