I think penny and Thomas did have a kid. And Thomas abandoned penny because of her mental health and ofc to marry Martha. He put Penny in arkham, and since he's rich, powerful and has a huge influence on Arkham hospital. So he would have made a fake document or certifrcate that claims Arthur is adopted, and not his. That would stain his reputation

first of all we viewers are all very clear about fantasy and reality in the movie there is no ambiguity about it

I liked that the movie didnt pretend that Thomas Wayne was a good guy. The comics always had something about Thomas that made him a rich bastard.

I believe there is some truth in what happened in the story. The biggest clue about this is: How would Arthur be able to recall the murder of Thomas Wayne if he wasnt there when it happened? Just food for thought
