Now give a name and click Create. Now if your form doesnt have any field to upload the files, You need to add a file upload field to your form. To add the field, click Insert Fields and choose File Upload. Now select the type of single file or multiple files and click Apply & Publish Step 3: Add The Form To Your Site First, you need to create a page. Give a name to your page and click the + icon. And search for a block Form and click on it. To add your page which youve created to your menu, Go to WordPress Dashboard Appearance Customize Now go to Menus Primary Menu Add Items Select the page and click Publish. To increase the maximum size for file upload, Go to WordPress Dashboard Forminator Forms Now click Edit on the form & click on the Upload File field Then go to Settings & under File Size Limit enter the maximum file size. Now, if someone submits a form, You will receive the details in your email along with the file theyve uploaded. To reply back to your visitors, Go to WordPress Dashboard Forminator Forms Now click Edit on the form which youve created. Go to Email Notifications Admin Email Advanced Now under Reply-to Email select Email Address and click Add. So, this how you can create a file upload form on your WordPress site

I had a few issues with making this look normal. In the end what worked for me was to open the page in elementor, add a text field with a designed background. To that i added a text field. In the text field i pasted the shortcode and finally after publishing it looked like in your video! :)

we want to have a plugin / form/ for wordpress website to keep our employees data. we want our staff can log in and fill various data in our form which can have many uploads for various data. then we can see it and can download in excel sheet. any suggestion. i have seen wp hrm,erp etc but all are paid. this form you shown is quite good.

Excelent Video tutorial.
I hope you can help me with my query. I am using your instructions regarding the Forminator form creation but I would like to force the user to select the specific file to download from a specific location. Not from my PC but from the server.
Can this be done?

I have tried the solution you provided in this video; but not succeeded fr me as I am getting an error at SMTP - Mailer: Other SMTP SMTP Error: data not accepted.

Is there a way to restrict the AMOUNT/number of files a member can upload first? So that after purchasing a membership they can upload more?
