That 12V connector. Yup, the one that Nvidia probably wanted their AIBs to move to and then didn't give them all the research data for the development of it, just a wild guess, so the AIBs didn't know you can't mount that connector at the top of a LARGE ASS GPU where the cable going into it is going to have to make a REALLY sharp bend in many PC cases which will put pressure on the connector, torque in fact which is terrible for solder joints or crimped connections, and now you have AIB GPUs that can be a fire hazard or at the very least scare people from being a DIYer after they smell smoke coming from their systems.
There's one way that's not safe to mount that 12 volt connector to the GPU PCB, and that's at the top of the board like they all did, except Nvidia who mounted it further down and at an angle so the adapter cable wouldn't be pushing up into the side panel of the PC case. I'm pretty sure Nvidia has the data saying WHY they mount that connector the way they did.

Kidding aside, he's right that we don't know definitively yet, but it does sound like a confluence of factors at different levels (industry group specification, nvidia design, factory production) contributed to the instances where the cable can fail, as well as any use/case-specific factors (e.g. bending the cable more or poorly seated connector, which normally should be anticipated with a good design and accounted for, or at least shouldn't lead to a runaway thermal situation aka melting the connector).
Otherwise it would be easier to identify the issue and more widespread/easier to reproduce, though I suspect the bad batch hypothesis around the adapters with 150V wires may be the most significant culprit in the equation.

Its not just now but its the second users of these cards that they need to think about now. What if someone purchases a used card a few months or years from now and the worst happens, the house burns down because of it?
It could be the way the cable is putting stress on the connection in certain builds due to the bend being put on the cable by mod builders who want it to look neat.

Props to Gordon for dealing with Tom at MLID for over 3 hours, I don't think I could stand a couple of minutes of talking with that guy. I still watch his videos for the information that may or may not come to fruition but as an individual the guy is horrible.

Gordon, people change. Intel didn't give us much for a decade and now Ryzen is the most sold CPU on Amazon. Nvidia has great cards but been screwing users with their prices for far too long. 4080s for 1600 in the EU? Nah, thank you.
