There’s a Dwarf in the first Dragon Age whose life you can totally ruin by doing any and all quests in Orzammar. His daughter can leave for the surface if you help her, you smash a whole in the wall of his smithy, the list goes on.

You didn't mention 'The Great Escape' from Borderlands 2. After all the effort you go through to allow Ulysses to escape, what happens afterwards is just tragic. Or comical, it is Borderlands after all.

I don’t think any NPC except maybe Roderika and Hewg has survived in their corresponding questline. Don’t even try to help an npc in Elden if you don’t wanna ruin their life lol
