Exaggerate much? We hear multiple stories of actors nearly dying on set yet the only actor I know who really died was Brandon Lee. My guess is that they get in a tiny bit of trouble and then exaggerate what happened. Stunt people on the other hand have died on and got injured horribly on set since unlike the stars they're the ones in REAL danger.

Another good example of a near on screen death is The Good, The Bad, And The Ugly.
During the scene where they blow up the bridge, you can see a massive stone fly right at Eastwood and land a few meters to his right, with how fast and how heavy that thing was it could have easily killed him if it hit.

An example could be the water cube trap in Saw. The actor had to literally had his head completly submerged. There was a signal if he couldnt make it, he couldve easily drowned. If he couldnt make it, they pulled a latch or something in the back causing the water to drain quickly.

3: 40 the one thing you missed was the point where Martin Sheen but his hand. That actual scene where he punched the mirror actually cut his hand. All that blood was his own. He figured he should just continue the scene as it was so they didn't need to do a retake so went with it.
