Actors Who Went Too Far For A Role Mark: Martin Sheen: Apocalypse Now - got drunk and insane for the first scene and cut his hand while punching a real mirror (not prop. Also, suffered a heart attack during filming.
Vincent D'Onofrio: Full Metal Jacket - gained 70 lbs for the role and injured his knee severely on the obstacle course scene. Filming took about 13 months to shoot and he hasn't really been skinny since.
Robert De Niro: Raging Bull - gained 60 lbs during shooting. It took 9 months to lose the weight again.
Daniel Day Lewis: several of his movies, especially when he plays the villain. As the bad guy in Gangs of New York and There Will Be Blood, he stayed in character and supposedly scared a few people so much that they quit the production altogether. He also got in scuffles at a local town. He refused to wear modern-day Winter clothing and got sick from the cold weather while filming Gangs.
Tobey Maguire had to do some quick body transformations during the 00's: Spider-Man movies, Seabiscuit, and Brothers.
Dustin Hoffman: Marathon Man - ran long distances during the day and stayed up late into the night. Insisted on making the torture scene more real by keeping him underwater for long periods. He needed to be given oxygen after 1 of the takes.
Nicolas Cage: Birdy - had teeth pulled while filming.
Renee Zellweger has had to do some quick body transformations too.
I'd say the deadly stunts Jackie Chan does for his movies are worth mentioning.
(I question whether the role of The Joker drove Heath Ledger insane. Many actors have disproven this theory. He was playing a different role in a different movie when he died)
How about forcing method acting on actors by a director? Stanley Kubrick kind of tortured Malcolm McDowell and Shelley Duvall during the filming of A Clockwork Orange and The Shining.

- Viggo Mortensen broke a bone in one of his toes in the scene they found where Merryl and Pippin were (LOTR 2. The moment he kneeled down was true and unexpected.
- Anthony Hopkins blinks very little in The Silence of the Lambs to look scarier.
- Leland Orsen (Se7en) didn't sleep for a couple of days to give his character that anxiety and nerves necessaey to be more realistic during the interview scene.

None of these compare to what Marlon Brando did to his costar, Maria Schneider, while filming Last Tango in Paris. And, if changing your weight is going too far you should include Robert DeNiro for his work in Raging Bull, where he gained 60 pounds and won an Oscar.

cmon heath ledger fing DIED because of drug use due to depression from his role. the guy used to lock himself away in a room and read comics and practice creepy smiles. he deserves to top this list; but goddammit he isnt even in it

Heath Ledger as the Joker and Jim Carrey as Andy Kaufman are good examples of actors who couldn't quite shake their character
