Just a suggestion for cooks new to the crock pot. I find that heavy cream will curdle and thin out to much if it is in the crockpot the majority of the cook time. I would use a little water, or bouillon/broth for the cook time, remove the chicken ladle out a lot of the rendered juices, and then whisk in the cream before returning the the chicken and adding the remaining ingredients. Turn it on low at that point. You can also add the cream and cheese at the very end of the cook.
