I feel the compulsion to state that, in Spec Ops: The Line, it's not a hallucination. It's a flashback from an unreliable narrator. You weren't hallucinating --- you were trying to search for some possible rationale for why all the horrible things you ended up doing were done. And it's a lot more comforting to think you were searching for him because he was doing these things than to think you were doing these things while searching for him.

I spent the whole of Soma 100%'convinced the whole game was actually just happening in your brain, as an exercise to help it heal from the brain tumor surgery your character is dying from. I was actually kind of disappointed when that didn't happen!

SH2 its still king of twists.
And Harvester should be there, I mean all a simulation that creates serial killers leaving you with the choice of living inside happy or going to the real life all messed up by killing your big love its messed up.

NieR didn't exactly seal humanity's fate. The canonical ending of Drakengard 1 started the White Chlorination with the destruction of the space alien...thing. But NieR's events do pretty much destroy all hope for mankind

Surprised not to see a nod to Shadow of the Colossus here - finding out that you're basically the bad guy and by killing all these majestic giants you've really just doomed yourself is pretty unsettling...

Bloodborne has got to be one of the best video games I have ever played.
To bad it's a ps4 exclusive because more people should try playing Bloodborne.
In my opinion it is the best soulsborn game so far.

Speaking of unsettling plot twists, maybe it-s obvious to some people but I highly recommend -Among the sleep-, check it out if you haven-t before. Loved that game.

Just wouldn't consider it -unsettling- so can't argue it not being on this list :p
